What the Assessment Produces

The Four Assessment Steps
- Problem Framing — Name the use case, describe the decision or workflow changing, define success in one sentence, and select a primary metric (time saved, cost per case, error rate, conversion rate, resolution time, or customer satisfaction)
- Exposure — Who is the primary user (employees, managers, customers, or public)? Where do AI outputs appear? What happens if the AI is wrong (annoying/recoverable, costly/reputational, legal/regulatory, or serious harm)?
- Autonomy — How much independent authority does the AI have? High-stakes flags checked?
- Opacity — What type of AI (predictive, generative, or agentic)? How traceable? What is the source of truth?
Risk Bands and What They Mean
- Score 1–8: "Pilot with speed" — Low risk. Move fast, document assumptions, and schedule the first quarterly AI Value Review.
- Score 9–16: "Add light guardrails" — Moderate risk. Ship with a human approval step, basic logging, and a clear fallback process.
- Score 17–32: "Active governance required" — High risk. Treat like a controlled system: formal risk sign-off, strong monitoring, access controls, and an incident response playbook.
- Score 33–64: "Executive approval required" — Very high risk. Requires an executive sponsor, compliance/legal review, independent adversarial testing, and strict human oversight.
The score is calculated as: Exposure × Autonomy × Opacity, each dimension scored 1–4.
